Agreement to Terms
By accessing or using the services of Hialeah MRI Center, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services. These terms apply to all patients, visitors, and users of our website and facilities.
Services Provided
Hialeah MRI Center provides medical imaging services, including MRI scans and related diagnostic services. All services are provided by qualified medical professionals and are subject to medical necessity and physician orders. We reserve the right to refuse service if medical criteria are not met or if patient safety cannot be ensured.
Appointments and Scheduling
Appointments are scheduled by appointment only. By scheduling an appointment, you agree to:
- Arrive on time for your scheduled appointment
- Provide accurate medical history and information
- Notify us at least 24 hours in advance if you need to cancel or reschedule
- Bring required documentation, including physician orders and insurance information
- Follow all pre-scan instructions provided by our staff
Payment and Billing
Payment is due at the time of service unless other arrangements have been made. We accept cash, credit cards, and most major insurance plans. You are responsible for understanding your insurance coverage and any out-of-pocket costs. Patients are responsible for any charges not covered by insurance, including deductibles, co-payments, and non-covered services.
Cancellation Policy
We require at least 24 hours notice for appointment cancellations. Late cancellations or no-shows may result in a cancellation fee. Repeated missed appointments may result in restrictions on future scheduling.
Medical Records and Results
All imaging results and reports are sent directly to your referring physician. You may request copies of your medical records in accordance with applicable laws. Results are typically available within 24-48 hours of your scan. We do not provide diagnostic interpretations directly to patients; please consult with your physician regarding your results.
Patient Responsibilities
As a patient, you are responsible for:
- Providing accurate and complete medical information
- Informing staff of any implants, devices, or contraindications to MRI
- Following all safety protocols and staff instructions
- Respecting staff, other patients, and facility property
- Maintaining confidentiality of other patients
Limitation of Liability
While we strive to provide the highest quality imaging services, we cannot guarantee specific outcomes or diagnoses. Our liability is limited to the direct cost of services provided. We are not responsible for delays, cancellations, or service interruptions due to equipment failure, emergencies, or circumstances beyond our control.
Changes to Terms
We reserve the right to modify these Terms of Service at any time. Changes will be effective immediately upon posting to our website. Your continued use of our services constitutes acceptance of any changes.
